War hammer, Flail or Knout?
Does it make a difference?
or is it just a cosmetic feature.

Just a warning, it doesn't work on war hammer. Only flail, knout, scourge and some of those staves. I'd go with a flail for low req.

HotO is a "caster" runeword, so it has to be made on a one-handed weapon (so you can use a shield) with very low str and dex requirements.

And 4 this reason, Flail is the best choice.
