Open Wounds is an ability based on a %. When it occurs, the target, monster or player, bleeds uncontrolably and loses HP. Depeneds on how much % you have for how much it bleeds though I think, not sure. www.battle.net/d2exp might be able to help you more with any related questions.

It will always bleed for 8 secs, and the damage is bassed on the clvl of the target.
