Reanimation items

I'm building a character based on creating armies out of fallen foes...what are some items you know of that give %chance to reanimate as _______?

So far I've got:

Skeleton archer soul (hell levels give 8% chance reanimate as)
Zombie soul (hell levels give 8% chance reanimate as)
Quill Rat soul (hell levels give 8% chance reanimate as)
Wild Boar soul (hell levels give 8% chance reanimate as)
Djinni soul (hell levels give 8% chance reanimate as)
Mage Eater soul (hell levels give 8% chance reanimate as)
Grave Digger soul (hell levels give 8% chance reanimate as)
The Smite of the Grave (unique Hellspawn skull, gives 2% reanimate as Lich)
We Are Nine (unique Mask, gives 127% chance reanimate as Plague Bearer)
Lady Lazarus (unique Teeth, gives % chance to reanimate as Devil Spider, don't remember exact %)

I may be wrong, but I thought Grade-A Nirvana, in addition to turning you INTO a cow, also gave you a % chance to reanimate as Hell Bovine...

Anything else you know of?

why would ya need this list: :P, this reminds me of having jamella for reg d2 and having 90 andys ****ing ruin my comp with lag!
