Creature Morphs

Anybody know of different items that let you morph into other creatures/monsters and stuff? I'm forming a bit of a collection. =)

So far I know of:

A handgun that gives the skill to morph to Venom Lord
Fallen souls give the skill to morph to Fallen
Unraveller/Radament souls automatically morph you into Radament upon equip
Rhino Beetle souls give ability to morph to Rhino Beetle
Overseer souls automatically morph you into an Overseer upon equip
Corpse Spitter souls automatically morph you into a Corpse Spitter upon equip
Quill Rat souls give ability to morph into Quill Rat
Sand Maggot souls automatically morph you into a Sand Maggot upon equip
Andariel Hidden Class automorphs you into Andariel
Duriel Hidden Class automorphs you into Duriel
Mephisto Hidden Class automorphs you into Mephisto
Diablo Hidden Class automorphs you into Diablo
Baal Hidden Class automorphs you into Baal
Grade-A Nirvana, a unique Thresher automorphs you into a Hell Bovine upon equip.
